In our practice you will find physicians and staff who care about you and your family. We love what we do and we do our best to keep you well. We listen to your concerns and make every effort to meet your health care needs. Welcome home.
A physician who has been trained in a specialty which provides continuing, comprehensive health care for the individual and family. The scope of family medicine encompasses all ages, both sexes, each organ system and every disease entity.
A physician with specialized training who promotes lifelong fitness and wellness, and encourages prevention of illness and injury. Dr. Hohman helps her patients maximize function and minimize disability and time away from sports, work, or school.
We pride ourselves in delivering comprehensive health care and that includes coordinating your tests, treatments, and referrals to specialists. In the age of heavily managed care, it is often necessary to get prior authorizations for CT scans, MRIs, or coverage for certain brand name medications that are not automatically covered by your health plan. Our medical assistants are experts at obtaining prior authorizations for imaging tests, medications, and referrals to certain specialists when needed.
We try our best to keep you out of the hospital, but if an ER visit or hospitalization is necessary we will communicate with the ER physician or hospitalist to notify them of your medical needs and to achieve a seamless transition back home. After a hospitalization, we will follow-up with you to ensure that your acute medical needs are met and any follow-up labs or specialist visits are coordinated promptly.
We have been using an electronic medical record system since 2004. This secure and robust system allows us to keep accurate information about your medical history at our fingertips. You will never need to worry about us losing your chart!
Using our electronic system, we are able to monitor and better manage care for our patients, promote patient safety while reducing costs, and improve overall patient health because of better continuity and coordination in patient care.
We are mindful of your finances and hope to provide cost-effective treatments without compromising quality of care. Our physicians routinely check to make sure prescribed medications are on your health plan’s formulary before electronically sending these prescriptions to your pharmacy. We often will choose generic medications instead of brand name medications if they are less expensive and have equal or better efficacy and safety. We encourage you to notify us of any financial limitations if and when they occur so that we can help better manage your medical expenses.
We have the ability to electronically send an accurate, error-free, and understandable prescription directly to your pharmacy from the point-of-care, which is an important element in improving the quality of patient care. Using our electronic system, we can easily fax or electronically send your prescriptions to your pharmacy of choice, which oftentimes will have your prescription ready when you get there.
Our patients who need blood tests appreciate the convenience of our blood drawing capabilities by our experienced medical assistants. This service can save you an extra trip to the lab as well as time waiting in line at other lab sites.
If you prefer, you can go to an off-site lab such as MuirLab, Quest Diagnostics, LabCorp or any local medical center for your tests. Please contact them directly for their locations/hours. Some of these sites offer Saturday hours that may be more convenient for your schedule. You should also make sure your chosen lab facility is a participating provider of your health plan.
